3/2/2024 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, mother discussed with QMHP-C on how Ashiyah struggles with being able to manage her anger when she cannot have her way. Mother reports isolation which behaviors can last up to 20 minutes in duration. Behavior: Ashiyah appeared to be a little upset at times within the session when discussing ways to be able to promote positive self-control at which she would smack her teeth at QMHP-C with behaviors last up to 2 minutes in duration. Intervention: QMHP-C explored positive behavior options with Ashiyah to support her willingness to make good decisions in the home. QMHP-C engaged in conversation with Ashiyah about positive decisions and her plan to continue building relationships with her mom and sibling. QMHP-C inquired about evening routine and after school activities Ashiyah is currently involved in. QMHP-C assisted Ashiyah in monitoring her own behaviors in efforts to promote exercising self-control in challenging areas. QMHP-C processed Ashiyah’s thoughts and feelings during self-reflection. QMHP-C supported Ashiyah in approaching her family with new and innovative ideas. QMHP-C continued to establish a positive supportive relationship with Ashiyah. QMHP-C will review and give feedback regarding ADHD and its causes and effect to well-being at home, as well as, his performance in the program. Intervention: QMHP-C reiterated the expectations and rules for the session. QMHP-C applauded Ashiyah for wanting to work on changing her behavior so that she leads by example and takes responsibility for her actions. QMHP-C reviewed Ashiyah after school schedule with her and supported as she prepped for the evening work she had. QMHP-C commended Ashiyah on utilizing creative strategies to increase her understanding of feelings and emotions as it relates to her mental illness. QMHP-C encouraged Ashiyah to continue displaying positive behaviors. QMHP-C praised Ashiyah for her cooperation. QMHP-C praised and recognized Ashiyah for her hard work, efforts, participation, and working towards improvement. QMHP-C praised Ashiyah for her commitment in trying to meet her objectives daily. Response: Ashiyah responds well to praise and support. Ashiyah implemented good communication skills by listening attentively. Ashiyah verbalized good thoughts and understanding. Ashiyah developed better communication skills by expressing herself in a calm manner. Ashiyah displayed management of individualized behaviors. Ashiyah has not overreacted with anger to minor frustrations or irritants. Ashiyah responded in a pleasant manner acknowledging the positive reinforcement that QMHP-C-C provided. Ashiyah was cooperative and compliant. Ashiyah displayed appropriate social behavior. Ashiyah was sociable and friendly while playing with her little sister. Ashiyah responds well to the positive reinforcement provided by QMHP-C and is seen increasing self-esteem and confidence. Ashiyah have adapted and is observed modeling appropriate behavior at this time. Response: Ashiyah expressed positive self-talk, modeled positive behavior, and good communication skills during role play activity. Ashiyah is making progress with expressing her feelings and showing others respect and courtesy by talking with appropriate tone, initiating conversation, and waiting for her turn to talk. Ashiyah responded well to feedback. Ashiyah communicated a positive attitude when reviewing problem solving and self-control strategies with her support system. Ashiyah expressed excitement, demonstrating mindfulness and seemed to be motivated to do the right thing. Ashiyah presents a positive attitude when given the opportunity to earn a reward. Ashiyah was happy and in a good spirit throughout the entire session. Ashiyah responded well to QMHP-C and appeared thankful for the extra help and support.
